Emergency Cards
Please make sure that you have a current emergency card on file in the school office. We also advise that you have a
release-for-treatment form on file authorizing your family physician and/or a local hospital to provide emergency
treatment in the event that you cannot be reached. You can indicate this information in the space provided on the
back of the school emergency card.
Medication
The following policies regarding medication are for the protection of all school children
  1. All medication (prescription and over the counter) must be brought to the school office.
  2. A written statement from the physician and signed by the parent, indicating the method, amount and time by
    which the medication is to be taken, must be provided.
  3. It is the student’s responsibility to come to the office to take the prescribed medication at the designated time.
  4. A parent or guardian can bring the prescribed medication to the school office and give it to their child directly.
Note:  Medication may not be kept in the classroom, lunch pail, or backpack.
Sickness
Out of consideration for everyone, please keep your child home if he/she is not feeling well. Students who have been ill
need to be fever free for 24 hours prior to returning to school.
Accidents
We take great care to see that John Adams is a safe place for students, staff and visitors. When accidents occur, they
must be reported immediately to the adult in charge. School personnel may administer first aid.
Accident Insurance
The school is not responsible for accidents that occur at school unless negligence is involved. In September of each
school year, parents have the opportunity to obtain low-cost accident insurance for their children through a
District-sponsored group plan. We urge you to make sure that your child is covered either by your personal insurance or
the District-sponsored plan. General liability insurance for parents and students during PTA-sponsored events is included
in your PTA membership.
