Library Permission Form
Our school Library is open to all students during their regularly scheduled class time.  Please encourage your child to
take advantage of this valuable resource.  Parent permission is required for students to “check out” books.
The library encourages good reading habits and provides students with an opportunity to select books of their
choice.  They will also gain the experience of being responsible for “checking out” books and returning them on
time.  Students of John Adams may check out books from the School Library weekly.
The Library Rules (Please read to your children to help them understand our Library rules.):
1.  Books are to be kept out for one week only and should be returned promptly.  Students who do not
return their book will not be allowed to check out another book.

2.  To be sure that the books receive proper care, we ask that the parents assume financial responsibility once
the book leaves the library.

3.  Parents are responsible for replacing lost books and books damaged beyond repair.  A book that is lost or
damaged beyond repair must be paid for, or be replaced with the same book.  If a book is found and returned
after being paid for, the payment will be refunded.

4.  Only one book may be checked out at a time unless otherwise authorized by the teacher.

5.  Students must conduct themselves in a quiet manner or they will be asked to leave.

6.  Children in grades K through 2 who would like to check out books, such as Goosebumps, Bone Chillers, or
other upper-grade scary books, must have a separate written permission slip from their parents.
